The undersigned acknowledges that this form, when completed and electronically signed, represents agreement to completing our third-party survey to measure impacts received by your company through LCTCS MEP of LA and our MEP affiliate organizations. It is our intention to impact your company in the following ways: new/retained jobs, new sales, retained sales, cost savings and new investments. Completing the survey allows for the LCTCS MEP of LA to continue operations and assist your company, your suppliers, and others in the manufacturing ecosystem. No company will be surveyed more than once/year after completion of a project. We appreciate your responses.
